Accurately complete and submit applications for patients seeking entry into DHC waiver programs, ensuring all paperwork is filled out correctly.
Follow up with patients' families to gather any missing information or documents needed for the application process.
Utilize organizational skills to manage multiple applications simultaneously, maintaining a high level of accuracy and attention to detail.
Operate within the company's system (training provided) to track and update application statuses and patient records.
The Profile Qualifications.
Background in healthcare, such as qualified nurses or doctors.
Bilingual, capable of communicating effectively with patients and their families.
Proficient in using Microsoft Word and Adobe for document management.
Highly organized with proven experience handling sensitive or confidential patient documents.
Independent worker who is persistent and dedicated to following up until all necessary documentation is received and processed.
Strong interpersonal skills and not just task-oriented; able to engage with families and patients compassionately and effectively.
Strong English writing skills is essential for this position.
This role requires a dedicated professional who is not only skilled in administrative tasks but also understands the nuances of patient care within the context of service coordination.
